These sample reports are actual Case Evaluation Reports based on real people and events.  These cases were sent to the Medical Review Foundation, Inc, from our Attorneys-Clients to investigate whether there was a breach from the standard of care.  This report, prepared by one of our Board Certified Medical Directors, includes a written analysis of the issues involving Negligence, Causation, Damages, and potential defendants and defenses. 

We hope that these many examples will assist you with all of your current cases, as well as showing you what you should expect to receive from anyone you choose to analyze each of your client's potential medical malpractice cases. 

The current, past and future Sample Case Evaluation Reports, referenced by its categories and titles on our website are consistently changing to also show you the range of medical malpractice issues we have investigated and exposed in 31 years in the medical-legal industry.  Over the next few weeks, we will be adding more sample cases, covering more topics of Medical Malpractice.  

We have taken extensive measures to protect the parties involved, at the same time being certain not to alter the facts and issues that surround each unique case.  

As you will see in each of these Case Evaluation Reports, the names of the parties involved (the patients, families, defendants, hospitals, nursing homes, physicians, nurses, etc), have all been changed and/or made anonymous for their protection.  In addition, dates and locations of where and when events occurred, have also been changed. 

Please note: Many Sample Case Reports relate to more than one speciality, but are listed under the a category we feel is most relevant.  On a few sample Case Evaluation Reports that relate to more than (3) three Medical Expert specialties, we have the sample case listed under the most important (2) two categories.  Therefore, it is recommended that you read all subject titles in the Table of Contents to identify all information of interest to you.
